Once you fill out your form, do the following:

1. Click the "Submit By e-mail" Button

2. Select the "Internet Mail" option

3. Click the "Save Data File" Button

4. You will then select a destination to which you'll save the data file, such as a folder or your desktop, then click "Save"

5. Go to your e-mail server, such as Yahoo, and send the data file to:
